Preparation
  1. Combine cocoa powder, sugar, and salt in the crockpot; whisk thoroughly.
  2. Gradually whisk in whole milk until smooth, then stir in heavy cream, vanilla, sweetened condensed milk, and chocolate chips.
  3. Cover and cook on low for 2-3 hours, stirring occasionally, until the chocolate is fully melted and the mixture is hot and creamy.
  4. Whisk again before serving warm, optionally garnished with whipped cream or marshmallows.

Notes

For an extra festive touch, add a dash of peppermint extract or a cinnamon stick while it cooks. This hot chocolate can be kept warm on the "warm" setting for up to 2 hours.
